Physician Competencies Needed To Pass The USMLE

The USMLE is the single most important tool that measures a physician’s skill and competencies. The following is a list of competencies that will be asked on the four USMLE Steps to guide where the medical student/ doctor should be at during that stage in training.  Each USMLE Step will question their abilities to think on their feet and perform at their maximum capabilities in the proper amount of time.

1. Medical knowledge and scientific concepts
a. Applying foundational science concepts

2. Patient diagnosis
a. Obtaining and predicting history and physical examination
b. Selecting and interpreting laboratory and diagnostic studies
c. Formulating the diagnosis
d. Determining prognosis and outcome

3. Patient care and management
a. Health maintenance and disease prevention
b. Selecting and monitoring pharmacotherapy
c. Selecting clinical interventions and treatment
d. Selecting clinical interventions and treatment

4. Communication and interpersonal skills
a. Monitoring and surveillance for disease recurrence and projection
b. Fostering the relationship
c. Gathering information
d. Providing information
e. Making decisions
f. Supporting emotions
g. Enabling patient behaviors
h. Using a translator

5. Professionalism, including legal and ethical issues
a. Understanding and applying principles of professionalism, legal, and ethical issues in interaction with patients
b. Understanding and applying legal and ethical issues related to death and dying
c. Understanding and applying professionalism, legal, and ethical issues in dealing with physicians and other health professionals

6. Systems-based practice, including patient safety
a. Complexity and systems thinking
b. Quality improvement
c. Patient safety
d. Health care policy and economics

7. Practice-based learning
a. Understanding and application of the principles of biostatistics and epidemiology
b. Make decisions about patient care based on results of study or other written material
c. Understands the principles of research ethics and regulatory issues
